Short answer
Every state Medicaid program covers GLP-1 medicines for type 2 diabetes. Covering them for weight management is optional: KFF counted 13 state programs doing so in January 2026, after California, New Hampshire, Pennsylvania and South Carolina ended coverage, and Massachusetts ended ongoing coverage in July 2026. The table shows every state.

What every Medicaid program must cover
Medicaid programs must cover nearly all FDA-approved medicines for their accepted medical uses, with a short list of optional exclusions that includes drugs for weight loss. So every state covers GLP-1s for type 2 diabetes, and states generally cannot exclude other approved uses, such as Wegovy to lower heart attack and stroke risk or Zepbound for obstructive sleep apnea, although they can require prior authorization. Only coverage for weight management on its own is a state choice.
What prior authorization usually requires
- A body mass index at or above the state’s threshold, sometimes with a weight-related condition such as high blood pressure or sleep apnea.
- Documentation of diet, activity or a structured weight-management program.
- For renewal, evidence of a minimum amount of weight lost by a set point.
- In some states, trying a preferred drug first, such as Wegovy before Zepbound or the reverse.
If your state does not cover it
You can ask whether another approved use applies to you, such as heart disease or sleep apnea, which Medicaid programs generally must cover with prior authorization. If a request is denied, you have the right to an appeal and a state fair hearing. Manufacturer copay cards cannot be used with Medicaid, but the manufacturers’ self-pay programs remain an option if you choose to pay yourself.
