Pharmacy rules in Hawaii
Hawaii lets pharmacists prescribe hormonal contraception, allows generic substitution unless the prescriber indicates otherwise, monitors controlled prescriptions through its Narcotics Enforcement Division, and had no state insulin cap in force when this guide was checked.

A Hawaii pharmacist may substitute a less expensive generic equivalent unless the prescriber has indicated on the prescription, or electronically, that the brand is required; Hawaii publishes a list of drugs that may not be substituted. The pharmacist must tell the patient about the substitution, and the patient may refuse it and pay for the brand. Interchangeable biological products may be substituted with notice to the prescriber.
Moving a prescription between pharmacies
Non-controlled prescriptions can be transferred between Hawaii pharmacies without a state limit on transfers. Refills are valid for a year from the date written. A pharmacist may dispense up to a ninety-day supply of a non-controlled maintenance medicine when the prescription allows. Check the board for the current electronic-prescribing requirements for controlled substances.
Controlled substances
Hawaii’s prescription monitoring program, run by the Department of Public Safety’s Narcotics Enforcement Division, collects every controlled dispensing and is consulted by prescribers before prescribing opioids and other controlled substances in many situations. Schedule II prescriptions cannot be refilled; Schedule III to V may be refilled up to five times within six months. Gabapentin was not scheduled in Hawaii when this guide was checked. A Schedule II prescription may be transferred electronically once under the federal rule.
What a pharmacist can prescribe or furnish
Hawaii pharmacists may prescribe hormonal contraception, including the pill, patch, ring and injection, under prescriptive authority in place since 2017, and may dispense naloxone, administer vaccines, and manage drug therapy under collaborative agreements. Each service depends on the pharmacy offering it, so ask.
Insulin and cost-sharing caps
Hawaii had no state insulin copay cap in force when this guide was checked. The federal Medicare Part D insulin cap applies to Part D plans, and manufacturer self-pay programs apply regardless of state. For cash-paying patients no cap applies.
Veterinary prescriptions
A veterinarian’s prescription for a human drug is filled by a Hawaii pharmacy under the same rules as any outside prescription. The Hawaii Board of Veterinary Medicine requires a veterinarian-client-patient relationship before prescribing, and veterinarians provide prescriptions on request so that clients can fill them elsewhere.
