Cash-paying patients, priced by you, paid at your counter
Patients who pay cash upload their prescription and ask pharmacies for a price. You see the request, quote what you want to charge, and if the patient chooses you, they collect and pay at your counter. There is no PBM in that sale.
Why cash-pay matters now
Federal PBM reform passed in February 2026, and several states have acted on their own, but most of the federal protections for pharmacies phase in from 2028 and apply to Medicare Part D rather than commercial plans. Meanwhile independent owners continue to describe being reimbursed below cost on many claims.
A cash sale is the one part of the business you control end to end: your acquisition cost, your price, payment at the counter, no network contract, no clawback or DIR on that transaction. Patients paying cash include the uninsured, people with high deductibles, people whose drug is excluded from their plan, and pet owners filling a veterinary prescription.
Patients are looking for a new pharmacy
Rite Aid closed every store, and chain closures have continued elsewhere, leaving patients whose files were moved to a pharmacy they did not choose, and some neighbourhoods with no pharmacy nearby. Many of those patients will move again if someone gives them a reason. A competitive cash price, quoted before they transfer anything, is a reason.
How it works for you
Requests from patients appear in your dashboard with the drug, strength, form and quantity from the prescription. You quote a price, or ignore the request. If the patient accepts your quote, an order is created with a pickup code; you mark it ready, the patient collects and pays you, and you mark it collected. Script Unlock charges a monthly subscription and a fee only on orders marked collected; quotes that are not accepted, cancellations and no-shows cost nothing.
You dispense only against a valid prescription received the way your state requires. The uploaded image lets you quote; it is not the prescription.
A good fit if
- You fill generics at a competitive acquisition cost and want more cash volume.
- You can turn around common prescriptions the same or next day.
- You want new patients who may also transfer their other prescriptions.
- You are willing to quote on veterinary prescriptions for human drugs.
Check before you quote
- How your PBM contracts define usual and customary price; some treat a published cash price list as U&C. Quoting case by case is different from publishing a list, but read your contracts.
- Your state’s rules on transfers, controlled substances and electronic prescribing, which our state guides summarize.
- That you can obtain the actual prescription, by e-prescription, transfer or original, before dispensing.
Questions pharmacies ask
Do patients pay through Script Unlock?
No. The patient pays you at collection, by whatever methods you accept. Script Unlock never handles the patient’s money.
What does it cost?
A monthly subscription and a fee on orders you mark as collected. The exact terms, and the founding-group terms while they last, are on the pricing page.
Can I see requests before deciding to quote?
Yes. You see the drug, strength, form and quantity and choose which requests to quote on. There is no obligation to quote.
How do you check pharmacies?
A person checks the pharmacy’s NPI record and state license before the account can see requests. We describe the process in full on the verification page.
Will this affect my PBM contracts?
Cash sales sit outside your PBM contracts, but some contracts define usual and customary price broadly. Check yours before setting cash prices, and ask your PSAO if you use one.
