A Colorado pharmacist may substitute a generic equivalent unless the prescriber has indicated “dispense as written” on the prescription or electronically. The pharmacist must tell the patient about the substitution, and the patient may refuse it and pay for the brand. Substitution follows the FDA’s therapeutic equivalence list, and interchangeable biological products may be substituted with notice to the prescriber.
Moving a prescription between pharmacies
Non-controlled prescriptions can be transferred between Colorado pharmacies without a state limit on transfers. Refills are valid for a year from the date written. A pharmacist may dispense up to a ninety-day supply of a non-controlled maintenance medicine when the prescription allows, and Colorado permits an emergency supply of certain maintenance medicines when a prescriber cannot be reached. Colorado has moved to require electronic prescribing of controlled substances; check the board for the current scope.
Controlled substances
Colorado’s Prescription Drug Monitoring Program must be checked by prescribers before prescribing most controlled substances, and pharmacies report every controlled dispensing. First opioid prescriptions for acute pain are limited to seven days, with exceptions. Schedule II prescriptions cannot be refilled; Schedule III to V may be refilled up to five times within six months. Gabapentin was not scheduled in Colorado when this guide was checked. A Schedule II prescription may be transferred electronically once under the federal rule.
What a pharmacist can prescribe or furnish
Colorado pharmacists may prescribe hormonal contraception after a screening, dispense naloxone under a standing order, administer vaccines, and under protocols may test for and treat some minor conditions and prescribe HIV prevention medicines; they may also manage drug therapy under collaborative agreements. Check that the pharmacy participates in each service.
Insulin and cost-sharing caps
Colorado was the first state to cap insulin cost-sharing on state-regulated health plans, in 2020, and has lowered the cap since; it does not apply to self-funded employer plans or to patients paying cash. Colorado also runs an insulin affordability programme that lets eligible residents buy insulin at a set price regardless of insurance; check the state’s page for eligibility. The federal Medicare Part D cap applies to Part D plans.
Veterinary prescriptions
A veterinarian’s prescription for a human drug is filled by a Colorado pharmacy under the same substitution and transfer rules. The Colorado State Board of Veterinary Medicine requires a veterinarian-client-patient relationship before prescribing, and Colorado law supports a client’s right to a written prescription so it can be filled elsewhere.
