An Arkansas pharmacist may substitute a less expensive generic equivalent unless the prescriber has indicated on the prescription, or electronically, that the brand is required. The pharmacist must tell the patient about the substitution, and the patient may refuse it and pay for the brand. Substitution follows the FDA’s therapeutic equivalence list, and interchangeable biological products may be substituted with notice to the prescriber.
Moving a prescription between pharmacies
Non-controlled prescriptions can be transferred between Arkansas pharmacies without a state limit on transfers. Refills are valid for a year from the date written. A pharmacist may dispense up to a ninety-day supply of a non-controlled maintenance medicine when the prescription allows. Arkansas has moved to require controlled-substance prescriptions to be sent electronically; check the board for the current scope.

The Arkansas Prescription Drug Monitoring Program must be checked by prescribers before prescribing opioids and benzodiazepines in most situations, and pharmacies report every controlled dispensing. Initial opioid prescriptions for acute pain are limited by state law to a short supply, with exceptions. Schedule II prescriptions cannot be refilled; Schedule III to V may be refilled up to five times within six months. Gabapentin was not scheduled in Arkansas when this guide was checked. A Schedule II prescription may be transferred electronically once under the federal rule.

Arkansas pharmacists may provide oral hormonal contraception and emergency contraception to adults under a statewide protocol in place since 2022; they may also dispense naloxone, administer vaccines, and manage drug therapy under protocols with physicians. Insulin and cost-sharing caps
Arkansas had no state insulin copay cap in force when this guide was checked. The federal Medicare Part D insulin cap applies to Part D plans, and manufacturer self-pay programs apply regardless of state. For cash-paying patients no cap applies.
