    Pharmacy rules in Oklahoma

    Oklahoma allows generic substitution unless the prescriber indicates the brand is required, limits first opioid prescriptions for acute pain to seven days, requires opioid prescriptions to be electronic, caps insulin cost-sharing on state-regulated plans, and does not let pharmacists prescribe contraception.

    Generic substitution

    An Oklahoma pharmacist may substitute a generic equivalent unless the prescriber has indicated on the prescription, or electronically, that the brand is required. The pharmacist must tell the patient about the substitution, and the patient may refuse it and pay for the brand. Substitution follows the FDA’s therapeutic equivalence list, and interchangeable biological products may be substituted with notice to the prescriber.

    Moving a prescription between pharmacies

    Non-controlled prescriptions can be transferred between Oklahoma pharmacies without a state limit on transfers. Refills are valid for a year from the date written. A pharmacist may dispense up to a ninety-day supply of a non-controlled maintenance medicine when the prescription allows. Oklahoma requires opioid prescriptions to be electronic, with exceptions, but not non-controlled prescriptions.

    Controlled substances

    Oklahoma’s Prescription Monitoring Program, run by the Bureau of Narcotics and Dangerous Drugs Control, must be checked by prescribers before prescribing opioids and other controlled substances in most situations. First opioid prescriptions for acute pain are limited to a seven-day supply, with exceptions. Schedule II prescriptions cannot be refilled; Schedule III to V may be refilled up to five times within six months. Gabapentin was not scheduled in Oklahoma when this guide was checked. A Schedule II prescription may be transferred electronically once under the federal rule.

    What a pharmacist can prescribe or furnish

    Oklahoma pharmacists may dispense naloxone without a prescription, administer vaccines, and manage drug therapy under protocols with physicians in defined settings. Oklahoma does not let pharmacists prescribe hormonal contraception. Services vary by pharmacy.

    Insulin and cost-sharing caps

    Oklahoma caps cost-sharing for insulin on state-regulated health plans at a fixed amount per thirty-day supply, with a matching cap for a ninety-day supply, under a 2021 law; it does not apply to self-funded employer plans or to patients paying cash. The federal Medicare Part D cap applies to Part D plans. For cash-paying patients no cap applies.

    Veterinary prescriptions

    A veterinarian’s prescription for a human drug is filled by an Oklahoma pharmacy under the same rules as any outside prescription; the electronic requirement for opioids applies to veterinarians with exceptions. The Oklahoma Board of Veterinary Medical Examiners requires a veterinarian-client-patient relationship before prescribing, and veterinarians provide prescriptions on request.
