A Kentucky pharmacist may substitute a less expensive generic equivalent unless the prescriber has indicated on the prescription, or electronically, that the brand must be dispensed. The pharmacist must tell the patient about the substitution, and the patient may refuse it and pay for the brand. Substitution follows the FDA’s therapeutic equivalence list, and interchangeable biological products may be substituted with notice to the prescriber.
Moving a prescription between pharmacies
Non-controlled prescriptions can be transferred between Kentucky pharmacies without a state limit on transfers. Refills are valid for a year from the date written. A pharmacist may dispense up to a ninety-day supply of a non-controlled maintenance medicine when the prescription allows. Kentucky has moved to require controlled-substance prescriptions to be sent electronically; check the board for the current scope and exceptions.
Controlled substances
Kentucky schedules gabapentin as a Schedule V controlled substance, so gabapentin prescriptions carry the refill, transfer and reporting rules of a controlled drug and are reported to KASPER, the state monitoring program, which prescribers must check before prescribing most controlled substances. Most first prescriptions of a Schedule II opioid for acute pain are limited to a three-day supply, with documented exceptions. Schedule II prescriptions cannot be refilled; Schedule III to V may be refilled up to five times within six months. A Schedule II prescription may be transferred electronically once under the federal rule.
What a pharmacist can prescribe or furnish
Kentucky pharmacists may dispense naloxone under protocol, administer vaccines, and manage drug therapy under collaborative care agreements with physicians. Pharmacist provision of contraception is limited in Kentucky to the progestin-only pill under a collaborative agreement, and that pill is now also sold without a prescription; other methods need a prescriber. Services vary by pharmacy.
Insulin and cost-sharing caps
Kentucky caps cost-sharing for insulin on state-regulated health plans at a fixed amount per thirty-day supply under a 2021 law; it does not apply to self-funded employer plans or to patients paying cash. Veterinary prescriptions
Gabapentin is a controlled substance in Kentucky, so a veterinary gabapentin prescription for a dog or cat has limited refills, cannot be transferred freely between pharmacies and is reported to KASPER under the owner; a pet owner who changes pharmacies may need a new prescription. The Kentucky Board of Veterinary Examiners requires a veterinarian-client-patient relationship before prescribing, and veterinarians provide prescriptions on request.
