    Pharmacy rules in Iowa

    Iowa gave pharmacists independent authority to prescribe hormonal contraception in 2025, requires controlled-substance prescriptions to be electronic, requires prescribers to register with and use its monitoring program, and had no state insulin cap in force when this guide was checked.

    Generic substitution

    An Iowa pharmacist may substitute a less expensive generic equivalent unless the prescriber has indicated on the prescription, or electronically, that the brand is required. The pharmacist must tell the patient about the substitution, and the patient may refuse it and pay for the brand. Substitution follows the FDA’s therapeutic equivalence list, and interchangeable biological products may be substituted with notice to the prescriber.

    Moving a prescription between pharmacies

    Non-controlled prescriptions can be transferred between Iowa pharmacies without a state limit on transfers. Refills are valid for a year from the date written. A pharmacist may dispense up to a ninety-day supply of a non-controlled maintenance medicine when the prescription allows. Iowa requires controlled-substance prescriptions to be sent electronically, with exceptions, but not non-controlled prescriptions.

    Controlled substances

    Iowa’s Prescription Monitoring Program collects every controlled dispensing, and prescribers must register with it and check it before prescribing opioids in most situations. Schedule II prescriptions cannot be refilled; Schedule III to V may be refilled up to five times within six months. Gabapentin was not scheduled in Iowa when this guide was checked. A Schedule II prescription may be transferred electronically once under the federal rule.

    What a pharmacist can prescribe or furnish

    Iowa pharmacists may prescribe hormonal contraception, including emergency contraception, under prescriptive authority that took effect in 2025; they may also dispense naloxone under protocol, administer vaccines, and manage drug therapy under collaborative agreements. Each service depends on the pharmacy offering it, so ask.

    Insulin and cost-sharing caps

    Iowa had no state insulin copay cap in force when this guide was checked. The federal Medicare Part D insulin cap applies to Part D plans, and manufacturer self-pay programs apply regardless of state. For cash-paying patients no cap applies.

    Veterinary prescriptions

    A veterinarian’s prescription for a human drug is filled by an Iowa pharmacy under the same rules as any outside prescription; the electronic requirement for controlled substances applies to veterinarians with exceptions. The Iowa Board of Veterinary Medicine requires a veterinarian-client-patient relationship before prescribing, and veterinarians provide prescriptions on request.
