Pharmacy rules in Oregon
Oregon was one of the first two states to let pharmacists prescribe hormonal contraception, allows generic substitution unless the prescriber marks no substitution, caps insulin cost-sharing on state-regulated plans, and gives pharmacists prescribing authority for a defined list of medicines and services.

An Oregon pharmacist may substitute a generic equivalent unless the prescriber has indicated “no substitution” or the equivalent on the prescription or electronically. The pharmacist must tell the patient about the substitution, and the patient may refuse it and pay for the brand. Substitution follows the FDA’s therapeutic equivalence list, and interchangeable biological products may be substituted with notice to the prescriber.
Moving a prescription between pharmacies
Non-controlled prescriptions can be transferred between Oregon pharmacies without a state limit on transfers. Refills are valid for a year from the date written. A pharmacist may dispense up to a ninety-day supply of a non-controlled maintenance medicine when the prescription allows, and Oregon permits an emergency refill of certain maintenance medicines when a prescriber cannot be reached. Check the board for the current electronic-prescribing requirements.
Controlled substances
Oregon’s Prescription Drug Monitoring Program collects every controlled dispensing, and prescribers are expected to consult it before prescribing opioids and other controlled substances. Opioid prescribing for acute pain follows state guidelines that favor short initial supplies. Schedule II prescriptions cannot be refilled; Schedule III to V may be refilled up to five times within six months. Gabapentin was not scheduled in Oregon when this guide was checked. A Schedule II prescription may be transferred electronically once under the federal rule.
What a pharmacist can prescribe or furnish
Oregon pharmacists may prescribe hormonal contraception, including the pill, patch, ring and injection, under a statewide protocol in place since 2016, and may prescribe other medicines and services on a list the board maintains, such as naloxone, smoking-cessation products and travel medicines; they also administer vaccines. Each service depends on the pharmacy offering it, so ask.
Insulin and cost-sharing caps
Oregon caps cost-sharing for insulin on state-regulated health plans at a fixed amount per thirty-day supply, with a matching cap for a ninety-day supply, under a 2024 law; it does not apply to self-funded employer plans or to patients paying cash. The federal Medicare Part D cap applies to Part D plans. For cash-paying patients no cap applies.
Veterinary prescriptions
A veterinarian’s prescription for a human drug is filled by an Oregon pharmacy under the same substitution and transfer rules. The Oregon Veterinary Medical Examining Board requires a veterinarian-client-patient relationship before prescribing, and veterinarians provide prescriptions on request.
