Pharmacy rules in Illinois
Illinois allows generic substitution unless the prescriber indicates the brand is required, caps insulin cost-sharing on state-regulated plans at one of the lowest levels in the country, requires controlled-substance prescriptions to be electronic, and regulates pharmacy through the Department of Financial and Professional Regulation.

An Illinois pharmacist may substitute a generic equivalent unless the prescriber has indicated that the brand is required, by writing “may not substitute” or the equivalent, or electronically. The pharmacist must tell the patient about the substitution and record the product dispensed. A patient may decline the substitution and pay for the brand. Interchangeable biological products may be substituted under similar rules with notice to the prescriber.
Moving a prescription between pharmacies
Non-controlled prescriptions can be transferred between Illinois pharmacies by pharmacist-to-pharmacist or electronic communication without a state limit on transfers. Refills are valid for a year from the date written. A pharmacist may dispense up to a ninety-day supply of a non-controlled maintenance medicine where the prescription allows. Illinois does not require non-controlled prescriptions to be electronic.
Controlled substances
Illinois requires controlled-substance prescriptions to be issued electronically, with exceptions, and prescribers must check the Illinois Prescription Monitoring Program before an initial opioid prescription. Schedule II prescriptions cannot be refilled; Schedule III to V prescriptions may be refilled up to five times within six months. Gabapentin is not scheduled in Illinois as of this guide’s date. Schedule II prescriptions may be transferred electronically once under the federal rule.
What a pharmacist can prescribe or furnish
Illinois pharmacists may dispense naloxone under a standing order, administer vaccines, and Illinois has moved to let pharmacists provide hormonal contraception under a standing order; check that the pharmacy participates. Collaborative practice agreements allow drug therapy management with a physician.
Insulin and cost-sharing caps
Illinois caps insulin cost-sharing on state-regulated health plans at a low fixed amount per thirty-day supply, reduced in 2024 from the original cap set in 2021; it does not apply to self-funded employer plans or to patients paying cash. The federal Medicare Part D cap applies to Part D plans. The state also runs an insulin safety-net programme for eligible residents facing an emergency need.
Veterinary prescriptions
A veterinarian’s prescription for a human drug is filled by an Illinois pharmacy under the same rules as any outside prescription; the electronic requirement for controlled substances applies to veterinarians with exceptions. Illinois requires a veterinarian-client-patient relationship before prescribing, and veterinarians provide prescriptions on request.
