A Missouri pharmacist may substitute a generic equivalent unless the prescriber has written “dispense as written” or the equivalent on the prescription or indicated it electronically. The pharmacist must tell the patient about the substitution, and the patient may refuse it. Substitution follows the FDA’s therapeutic equivalence list, and interchangeable biological products may be substituted with notice to the prescriber.
Moving a prescription between pharmacies
Non-controlled prescriptions can be transferred between Missouri pharmacies without a state limit on transfers. Refills are valid for a year from the date written. A pharmacist may dispense up to a ninety-day supply of a non-controlled maintenance medicine when the prescription allows. Missouri has moved towards electronic prescribing of controlled substances; check the board for the current scope and exceptions.
Controlled substances
Missouri’s statewide prescription drug monitoring program, authorised in 2021 and launched afterwards, replaced the county-based program that had covered most of the state; prescribers and pharmacists report and may consult it. First opioid prescriptions for acute pain are limited to seven days, with exceptions. Schedule II prescriptions cannot be refilled; Schedule III to V may be refilled up to five times within six months. Gabapentin was not scheduled in Missouri when this guide was checked. A Schedule II prescription may be transferred electronically once under the federal rule.
What a pharmacist can prescribe or furnish
Missouri pharmacists may dispense naloxone under a statewide standing order, administer vaccines, and manage drug therapy under collaborative agreements with physicians. Missouri had not granted pharmacists general authority to prescribe hormonal contraception when this guide was checked. Services vary by pharmacy.
Insulin and cost-sharing caps
Missouri had no state insulin copay cap in force when this guide was checked. The federal Medicare Part D insulin cap applies to Part D plans, and manufacturer self-pay programmes apply regardless of state. For cash-paying patients no cap applies.

A veterinarian’s prescription for a human drug is filled by a Missouri pharmacy under the same rules as any outside prescription. The Missouri Veterinary Medical Board requires a veterinarian-client-patient relationship before prescribing, and veterinarians provide prescriptions on request so that clients can fill them elsewhere.
