The pharmacist must tell the patient about the substitution, and the patient may refuse it and pay for the brand. Non-controlled prescriptions can be transferred between West Virginia pharmacies without a state limit on transfers. Refills are valid for a year from the date written. First opioid prescriptions are limited by state law to a short supply, shorter in emergency departments and urgent care than in other settings. Schedule II prescriptions cannot be refilled; Schedule III to V may be refilled up to five times within six months. A Schedule II prescription may be transferred electronically once under the federal rule.

West Virginia pharmacists may provide hormonal contraception, including the pill, patch, ring and emergency contraception, to adults under a standing order in place since 2020, and may dispense naloxone, administer vaccines, and manage drug therapy under collaborative agreements. Each service depends on the pharmacy offering it, so ask.
Insulin and cost-sharing caps
West Virginia caps total cost-sharing for insulin on state-regulated health plans at a fixed amount per thirty-day supply, and separately caps diabetes equipment and supplies, under a 2023 law; it does not apply to self-funded employer plans or to patients paying cash. The federal Medicare Part D cap applies to Part D plans.
Veterinary prescriptions
Gabapentin is a controlled substance in West Virginia, so a veterinary gabapentin prescription for a dog or cat has limited refills, cannot be transferred freely between pharmacies and is reported to the monitoring program under the owner; a pet owner who changes pharmacies may need a new prescription.
