A Rhode Island pharmacist may substitute a less expensive generic equivalent unless the prescriber has indicated on the prescription, or electronically, that the brand is required. The pharmacist must tell the patient about the substitution, and the patient may refuse it and pay for the brand. Substitution follows the FDA’s therapeutic equivalence list, and interchangeable biological products may be substituted with notice to the prescriber.
Moving a prescription between pharmacies
Non-controlled prescriptions can be transferred between Rhode Island pharmacies without a state limit on transfers. Refills are valid for a year from the date written. A pharmacist may dispense up to a ninety-day supply of a non-controlled maintenance medicine when the prescription allows. Controlled substances
Rhode Island’s Prescription Drug Monitoring Program, run by the Department of Health, must be checked by prescribers before prescribing opioids and other controlled substances in most situations. State regulations limit first opioid prescriptions for acute pain to a small supply and daily dose for most patients. Gabapentin was not scheduled in Rhode Island when this guide was checked. Schedule II prescriptions cannot be refilled; Schedule III to V may be refilled up to five times within six months. A Schedule II prescription may be transferred electronically once under the federal rule.

Rhode Island has authorized pharmacists to provide hormonal contraception under a statewide protocol; check the board for when and how the protocol is in effect. Pharmacists may also dispense naloxone, administer vaccines, and manage drug therapy under collaborative agreements. Services vary by pharmacy.
Insulin and cost-sharing caps
Rhode Island caps cost-sharing for insulin on state-regulated health plans at a fixed amount per thirty-day supply under a 2021 law; it does not apply to self-funded employer plans or to patients paying cash. The federal Medicare Part D cap applies to Part D plans. For cash-paying patients no cap applies.
