Pharmacy rules in Montana
Montana gives pharmacists prescriptive authority for hormonal contraception, caps insulin cost-sharing on state-regulated plans, requires a check of its monitoring program before many controlled prescriptions, and allows generic substitution unless the prescriber indicates otherwise.

A Montana pharmacist may substitute a less expensive generic equivalent unless the prescriber has indicated on the prescription, or electronically, that the brand is required. The pharmacist must tell the patient about the substitution, and the patient may refuse it and pay for the brand. Substitution follows the FDA’s therapeutic equivalence list, and interchangeable biological products may be substituted with notice to the prescriber.
Moving a prescription between pharmacies
Non-controlled prescriptions can be transferred between Montana pharmacies without a state limit on transfers. Refills are valid for a year from the date written. A pharmacist may dispense up to a ninety-day supply of a non-controlled maintenance medicine when the prescription allows. Controlled substances
The Montana Prescription Drug Registry collects every controlled dispensing, and prescribers must check it before prescribing opioids and benzodiazepines in most situations. Opioid prescribing for acute pain follows state rules that favor short initial supplies. Gabapentin was not scheduled in Montana when this guide was checked. Schedule II prescriptions cannot be refilled; Schedule III to V may be refilled up to five times within six months. A Schedule II prescription may be transferred electronically once under the federal rule.
What a pharmacist can prescribe or furnish
Montana pharmacists may prescribe hormonal contraception under prescriptive authority in place since 2023, and may dispense naloxone, administer vaccines, and manage drug therapy under collaborative agreements. Each service depends on the pharmacy offering it, so ask.
Insulin and cost-sharing caps
Montana caps cost-sharing for insulin on state-regulated health plans at a fixed amount per thirty-day supply; it does not apply to self-funded employer plans or to patients paying cash. The federal Medicare Part D cap applies to Part D plans. For cash-paying patients no cap applies.
Veterinary prescriptions
A veterinarian’s prescription for a human drug is filled by a Montana pharmacy under the same rules as any outside prescription. The Montana Board of Veterinary Medicine requires a veterinarian-client-patient relationship before prescribing, and veterinarians provide prescriptions on request so that clients can fill them elsewhere.
