About Spironolactone
Spironolactone blocks aldosterone, a hormone that makes the kidneys retain sodium and water and lose potassium. Blocking it removes fluid while keeping potassium, and at higher doses it also blocks androgen receptors, which is why it is used for hormonal acne and hirsutism in women and as part of some hormone therapy regimens.
In heart failure it reduces hospitalisations and deaths at low doses. It is also used for resistant high blood pressure, for fluid from liver disease, and for primary hyperaldosteronism. Because it raises potassium, blood tests are checked after starting and with any dose change, and potassium supplements or salt substitutes are usually avoided.
Breast tenderness or enlargement, menstrual changes and, in men, gynaecomastia are the recognizable side effects. It carries a warning based on tumor findings in rodents at high doses, which is why prescribers use the lowest effective dose.
