    Hair Loss: medications and paying cash

    Pattern hair loss is treated with minoxidil and, in men, finasteride or dutasteride; treatment is long term and almost always paid for in cash.

    Overview

    Androgenetic alopecia causes gradual thinning at the temples and crown in men and a widening parting in women, driven by genetics and the effect of androgens on hair follicles. Sudden shedding, patchy loss, scarring or loss with other symptoms points to a different cause, such as thyroid disease, iron deficiency, alopecia areata, a recent illness or a medicine, which is treated differently, so it is worth a medical assessment first.

    Minoxidil, as a topical solution or foam or as a low-dose tablet used off-label, prolongs the growth phase of follicles. Finasteride, and off-label dutasteride, block the conversion of testosterone to dihydrotestosterone and slow or partly reverse thinning in men; they are not used by women who could become pregnant. For women, spironolactone is used off-label, often with minoxidil. Results take six to twelve months, and hair regained is lost within months of stopping, so treatment is long term.

    Alopecia areata, an autoimmune cause of patchy loss, now has approved JAK-inhibitor tablets, which are specialist brand medicines.

    Choosing between the options when you pay cash

    The generic medicines here are among the cheapest a pharmacy dispenses: finasteride, minoxidil tablets and spironolactone cost pharmacies very little, and a ninety-day prescription at a flat-price pharmacy is a small outlay. Topical minoxidil is an over-the-counter product, cheapest as a store brand. Dutasteride costs more than finasteride.

    The expensive part is usually the channel, not the drug: hair-loss telehealth services bundle a consultation with the medicine at a monthly price many times the pharmacy cost. If a service will send the prescription to a pharmacy of your choice, the saving is large.

    When paying cash beats a copay, and when it does not

    Insurance rarely covers pattern hair loss, so cash is the route. A ninety-day prescription filled at a flat-price or independent pharmacy, plus store-brand topical minoxidil, is the cheapest way to stay on treatment long term.
