About Celecoxib

Celecoxib is believed to work mainly by blocking cyclooxygenase-2 (COX-2), an enzyme that makes prostaglandins, signals behind pain and inflammation. Capsules are labeled for osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, acute pain in adults, menstrual cramps (primary dysmenorrhea) and, from age 2, juvenile rheumatoid arthritis. Osteoarthritis is treated with 200 mg a day, as one dose or 100 mg twice a day, and rheumatoid arthritis with 100 to 200 mg twice a day; for acute pain or cramps, 400 mg is followed by 200 mg more on the first day if needed, then 200 mg twice a day as needed. Children take 50 mg twice a day from 10 to 25 kg and 100 mg twice a day above 25 kg. Moderate liver impairment halves the dose, use with severe impairment is not recommended, and people known or suspected to clear it slowly (CYP2C9 poor metabolizers) start at half the lowest dose.

Its boxed warning says NSAIDs raise the risk of serious heart attacks and strokes, which can be fatal and may begin early in treatment and grow with longer use, and of bleeding, ulcers and perforation of the stomach or intestines, which can be fatal, can occur at any time without warning, and are more likely in older adults and people with past ulcers or bleeding. Celecoxib must not be used in the setting of coronary artery bypass graft (CABG) surgery, by anyone allergic to it, after asthma, hives or an allergic-type reaction to aspirin or another NSAID, or after an allergic-type reaction to sulfonamides. Other warnings cover liver injury, high blood pressure, heart failure and swelling, kidney damage and high potassium, severe allergic and skin reactions including DRESS, anemia and masked signs of infection. Abdominal pain, diarrhea, indigestion, gas, swelling, dizziness and upper respiratory infections are among the most common side effects.

Elyxyb, a 25 mg per mL solution, is labeled for the acute treatment of migraine with or without aura in adults: one 120 mg dose, with or without food, and no more than 120 mg in 24 hours; it does not prevent migraine. Vyscoxa, a 10 mg per mL suspension approved in July 2025, has the arthritis uses, including juvenile arthritis from age 2, but not acute pain or menstrual cramps; it is taken on an empty stomach, and its label caps a single dose at 200 mg. From about 20 weeks of pregnancy, NSAIDs can cause rare but serious kidney problems in the unborn baby and low amniotic fluid, and from about 30 weeks can close a fetal blood vessel too early, so the label limits use between about 20 and 30 weeks and says to avoid it after that. Seglentis (with tramadol) and Consensi (with amlodipine) are discontinued.

Taking Celecoxib

How to take it

Capsules once or twice a day, with or without food; MedlinePlus says doses above 200 mg are taken with food. If capsules are hard to swallow, empty one onto a level teaspoon of applesauce and swallow it at once with water. Take Vyscoxa on an empty stomach, and measure a reduced Elyxyb dose with a calibrated device, never a household spoon.

If you miss a dose

Take a missed dose as soon as you remember, unless the next is almost due; then skip it and keep to the regular schedule. Do not take a double dose.

Storage

At room temperature in the original, tightly closed container, away from heat and moisture, out of reach of children. Do not refrigerate or freeze Elyxyb; discard what is left in an opened bottle. Use a take-back program rather than flushing.

Interactions to know about

Pain-relieving doses of aspirin and other NSAIDs are not recommended, and it does not replace low-dose aspirin for heart protection. Blood thinners, SSRIs and SNRIs add to bleeding risk, and corticosteroids to stomach risk. It can weaken ACE inhibitors, ARBs, beta blockers and diuretics, raise digoxin and lithium levels, and add to methotrexate and cyclosporine toxicity. Fluconazole raises its levels, rifampin lowers them, and it can raise levels of CYP2D6 drugs such as atomoxetine.

Why the cash price for Celecoxib differs from pharmacy to pharmacy

Generic celecoxib capsules are rated equivalent to Celebrex in every strength, so quotes for the same strength differ mainly by each pharmacy’s fee and markup and by whether celecoxib is on its flat-price generic list. A prescription that names Celebrex and blocks substitution is priced as a brand.

Elyxyb and Vyscoxa are brand-only liquids with no rated equivalent, so neither can be exchanged for capsules or for each other at the counter.

Ways to pay less for Celecoxib without insurance

• Ask for generic capsules, check that the prescription allows substitution, and compare more than one pharmacy.
• Ask whether celecoxib is on the pharmacy’s flat-price generic list, and for a ninety-day supply once a long-term dose is settled.
• If Vyscoxa is prescribed because capsules are hard to swallow, ask the prescriber whether capsules opened onto applesauce would suit you.
• For Elyxyb or Vyscoxa, ask the maker whether a savings program applies to cash payers; many copay cards require commercial insurance.
