Quitting Smoking: medications and paying cash
Varenicline, bupropion and nicotine replacement each make quitting considerably more likely than trying without medicine, especially alongside counseling.
Overview
Nicotine dependence keeps people smoking long after they want to stop, and most people who quit successfully have tried several times before. Planning a quit date, preparing for triggers, and using support from a quit line, an app or a counsellor all improve the odds, and so do medicines.
Varenicline is the most effective single medicine; it eases cravings and makes smoking less rewarding, and is started a week before the quit date for a twelve-week course. Bupropion, an antidepressant, is an alternative started one to two weeks before quitting. Nicotine replacement, as patches combined with a faster form such as gum or lozenges, is as effective as varenicline when used together and is sold over the counter.
Combinations are sometimes used, such as varenicline with nicotine replacement, under medical advice. Vaping is not an approved quitting aid in the United States, and the evidence for it is still developing.
Choosing between the options when you pay cash
Bupropion SR is a cheap generic with a cash price that is mostly dispensing fee. Generic varenicline costs pharmacies more, and pharmacies price the starter pack, the continuing pack and plain bottles differently, so quotes for a full course vary widely. Over-the-counter nicotine patches, gum and lozenges are cheapest as store brands, and combination use doubles the number of products to buy.
The choice between them is clinical first: varenicline for most smokers, bupropion when depression is also present or varenicline is not tolerated, combination nicotine replacement for anyone who prefers not to take a prescription medicine.
When paying cash beats a copay, and when it does not
Before paying cash, check your plan: many insurers cover smoking-cessation medicines, and sometimes over-the-counter nicotine replacement with a prescription, at no cost to you. Without coverage, comparing two pharmacies for a twelve-week varenicline course and buying store-brand nicotine replacement keeps the cost down.
