Overview
Generalized anxiety, panic disorder and social anxiety are common, treatable and often long-lasting. Talking therapy is as effective as medication for many people and the two are often combined; medication is usually suggested when symptoms are persistent or severe.
The SSRIs sertraline and escitalopram and the SNRI venlafaxine are first-line daily treatments; they take several weeks to work and are continued for months after improvement. Buspirone treats generalized anxiety without sedation or dependence but also needs weeks. Hydroxyzine, an antihistamine, works within an hour for acute anxiety and is not controlled. Benzodiazepines such as alprazolam and lorazepam work quickly but cause tolerance and dependence, so they are used for short periods or specific situations.
Side effects and the patient’s other conditions guide the choice between SSRIs, which differ more in tolerability than in effect.
Choosing between the options when you pay cash
All the first-line antianxiety medicines are commodity generics, so the clinical choice should lead and the cash price is mostly dispensing fee. Sertraline, escitalopram, citalopram and buspirone appear on most flat-price lists. Venlafaxine is the one to watch: the extended-release capsule is cheap, the extended-release tablet is a different product that costs far more, and a prescription written for one cannot be filled with the other.
Benzodiazepines are also cheap generics, but as controlled substances they cannot be transferred freely between pharmacies and some pharmacies limit quantities; a cash-paying patient should expect the dispensing fee rather than the drug to set the price.
When paying cash beats a copay, and when it does not
For sertraline, escitalopram, citalopram, buspirone and hydroxyzine, a cash quote from an independent or mail-order pharmacy is commonly at or below a copay, and ninety-day fills make it clearly cheaper. Insurance rarely changes the picture unless the prescription is a brand or the venlafaxine tablet form.
