    DexmethylphenidateDexmethylphenidate Hydrochloride

Dexmethylphenidate (Focalin tablets and Focalin XR capsules) is a stimulant for ADHD; the tablets were studied in children 6 to 17, and Focalin XR in children 6 and older and in adults. It is Schedule II, so every fill needs a new prescription, and carries a boxed warning about abuse, misuse and addiction. Every strength of both forms has generics rated equivalent, and both brands are still sold.

    About Dexmethylphenidate

Dexmethylphenidate is one of the two mirror-image forms that make up methylphenidate (Ritalin and others), so someone switching from methylphenidate starts at half their total daily dose. Focalin tablets are labeled for ADHD, with safety and effectiveness established in children 6 to 17; those new to methylphenidate start at 2.5 mg twice a day, at least 4 hours apart, raised weekly by 2.5 to 5 mg to at most 20 mg a day. Focalin XR, taken once each morning, starts at 5 mg in children and 10 mg in adults, raised weekly by 5 or 10 mg; doses above 30 mg a day in children or 40 mg in adults have not been studied and are not recommended. Someone on the tablets can switch to the same daily dose of Focalin XR, and the label calls for stopping if there is no improvement after a month of dose adjustment.

The boxed warning says it has a high potential for abuse and misuse that can lead to a substance use disorder, including addiction, and that misuse can cause overdose and death, so prescribers assess this risk before and during treatment. Other warnings cover serious heart disease, raised blood pressure and heart rate, psychotic or manic symptoms, painful or prolonged erections, poor circulation in the fingers and toes, slowed growth in children, angle-closure glaucoma and raised eye pressure, and tics. It must not be taken with or within 14 days of an MAO inhibitor. Abdominal pain, fever, loss of appetite and nausea were most common with the tablets in children; with Focalin XR, indigestion, decreased appetite, headache and anxiety in children, and dry mouth, indigestion, headache, sore throat and anxiety in adults.

Each Focalin XR capsule holds two kinds of beads, half released at once and half later; it is swallowed whole or opened and all of it sprinkled on applesauce, and the contents of one capsule are never divided, chewed or crushed. The Focalin XR label, revised in September 2025 after a 2025 FDA safety communication, says it is not recommended under age 6, where children had higher drug levels and more side effects, including weight loss; the tablets have not been shown to be safe and effective under 6. As a Schedule II drug it cannot be refilled. Dexmethylphenidate was not on the FDA shortage list in October 2026, though some extended-release methylphenidate products were.

    Taking Dexmethylphenidate

    How to take it
Tablets: twice a day, at least 4 hours apart, with or without food. Focalin XR: once each morning, with or without food, swallowed whole, or opened and the whole contents sprinkled on a spoonful of applesauce and swallowed right away without chewing; do not save the mixture.
    If you miss a dose
MedlinePlus says to take it when you remember unless it is almost time for the next dose, then skip it. Do not take a double dose.
    Storage
In a safe place, preferably locked, in a tightly closed container at room temperature. Do not flush it; use a medicine take-back program.
    Interactions to know about
Never with or within 14 days of an MAO inhibitor. It can weaken blood pressure medicines; it is avoided on the day of surgery with halogenated anesthetics, because blood pressure and heart rate can rise suddenly; and changing the dose of it or of risperidone can bring on movement side effects.

This summarizes the official prescribing information. Your prescriber’s and pharmacist’s instructions, and the leaflet that comes with your medicine, take priority.

Why the cash price for Dexmethylphenidate differs from pharmacy to pharmacy

Both forms have several generic makers rated equivalent to the brand, so quotes differ mainly by fee and markup, unless the prescription names Focalin or Focalin XR and blocks substitution, which prices it as a brand. Focalin XR comes in eight strengths, and a pharmacy may stock only some of them, so an unusual strength may need to be ordered.

Schedule II drugs are ordered on a special DEA form from wholesalers that must flag unusual orders and can set their own thresholds, so a pharmacy may fill only part of a prescription or decline it.

Ways to pay less for Dexmethylphenidate without insurance

    • Ask for generic substitution, and make sure the prescription does not name the brand only.
    • If your Focalin XR dose is made of two capsules, ask the prescriber whether one capsule strength could give the same dose.
    • Confirm stock before the prescription is sent, since an unfilled electronic prescription can be transferred only once.
    • Ask for a partial fill if you want fewer at first; where state law allows, the rest can be collected within 30 days of the date written.
    • Upload the prescription to Script Unlock so licensed pharmacies can quote their cash price. You pay the pharmacy directly.
