Anticonvulsants

    Topiramate

Topiramate (Topamax; extended-release Trokendi XR and Qudexy XR; Eprontia oral solution) is an anticonvulsant labeled for epilepsy from age 2 (age 6 for Trokendi XR) and for preventing migraine from age 12. Every form has a generic, but the two extended-release capsules are not interchangeable. It can cause birth defects such as cleft lip and palate, and sudden eye pain or blurred vision needs urgent care.

    About Topiramate

Topiramate is an anticonvulsant; MedlinePlus says it works by decreasing abnormal excitement in the brain. Topamax tablets and sprinkle capsules, Qudexy XR, Eprontia and their generics are labeled for epilepsy from age 2, alone (partial-onset or primary generalized tonic-clonic seizures) or as an add-on (also for Lennox-Gastaut syndrome); Trokendi XR is labeled for epilepsy from age 6. All are labeled to prevent migraine from age 12, not to treat an attack. Doses rise weekly: for migraine, from 25 mg each evening to 50 mg twice a day in week four; for epilepsy alone, to 200 mg twice a day over six weeks in adults and children 10 and older. As an add-on, adults usually take 200 to 400 mg a day, and children are dosed by weight. Extended-release capsules give the daily dose once a day, and reduced kidney function can call for half the usual adult dose.

Topiramate has no boxed warning. It can cause sudden nearsightedness and secondary angle-closure glaucoma, usually within the first month, with blurred vision or eye pain; it is stopped quickly, since untreated eye pressure can cause permanent vision loss. Other warnings cover visual field loss; reduced sweating and overheating, especially in children in hot weather; metabolic acidosis, checked with blood tests; suicidal thoughts and behavior; slowed thinking, word-finding problems, mood changes and sleepiness; slower bone mineral gain and growth in children; serious skin reactions; kidney stones; high ammonia levels; and low body temperature with valproate. A March 2026 safety update revised the warnings on DRESS, anaphylaxis and angioedema and made a past allergic reaction to topiramate a contraindication, and in August 2026 the Qudexy XR label added a warning, based on laboratory findings for topiramate, of serious heart rhythm and conduction problems, especially with heart disease or arrhythmias. Tingling, poor appetite, weight loss, speech and memory problems, tiredness and dizziness are common.

Exposure in pregnancy raises the risk of major birth defects, including cleft lip and palate, which form in the first trimester, and of babies small for their age; women who could become pregnant weigh this with the prescriber and, if not planning a pregnancy, use effective contraception. Topiramate can make estrogen-containing birth control less effective and cause breakthrough bleeding, and it passes into breast milk. Trokendi XR is swallowed whole, with no alcohol from 6 hours before to 6 hours after each dose. Qsymia (phentermine with topiramate) has its own guide. Stopping suddenly can bring on seizures, so doses are lowered gradually.

    Taking Topiramate

    How to take it
With or without food. Swallow tablets whole, since they taste bitter. Sprinkle capsules and Qudexy XR may be swallowed whole or opened over a teaspoon of soft food and swallowed at once without chewing; swallow Trokendi XR whole. Drink plenty of fluids to help prevent kidney stones.
    If you miss a dose
Tablets, sprinkle capsules or solution: take it when you remember, but skip it if the next dose is due in less than 6 hours. Extended-release capsules: take it unless the next dose is nearly due, and call the prescriber if you miss more than one. Never take a double dose.
    Storage
At room temperature (sprinkle capsules at or below 77°F), away from heat and moisture, out of reach of children. Discard unused solution 90 days after first opening.
    Interactions to know about
Valproate with topiramate can cause high ammonia levels and low body temperature; other carbonic anhydrase inhibitors such as zonisamide and acetazolamide add to the risk of acidosis and kidney stones; phenytoin and carbamazepine lower its levels; lithium is checked at high doses; and alcohol and other sedatives add to drowsiness.

This summarizes the official prescribing information. Your prescriber’s and pharmacist’s instructions, and the leaflet that comes with your medicine, take priority.

Why the cash price for Topiramate differs from pharmacy to pharmacy

Extended-release capsules come in two families, Trokendi XR and Qudexy XR, each with its own generics, and a pharmacist can substitute only a generic rated to the product written; a prescription that names a brand and blocks substitution is priced as a brand.

Generic tablets have many makers and sprinkle capsules several; the oral solution has fewer, and an opened bottle is discarded after 90 days.

Ways to pay less for Topiramate without insurance

    • Ask for generic topiramate, and check that the prescription does not name a brand only.
    • If extended-release capsules are prescribed, ask the prescriber whether twice-daily generic tablets would suit you.
    • Ask for quotes on your dose made up of different tablet strengths, and ask the prescriber which to write.
    • Once your dose is settled, ask for a ninety-day supply and whether topiramate is on the pharmacy’s flat-price generic list.
    • Upload the prescription to Script Unlock so licensed pharmacies can quote their cash price. You pay the pharmacy directly.
