    Anxiety in dogs

    Separation anxiety, noise phobia and fear of visits, travel or strangers are common and treatable. Behaviour modification is the core; medicines support it, from as-needed trazodone and gabapentin to daily fluoxetine or clomipramine. Nearly all of them are inexpensive human generics a pharmacy fills on a vet’s prescription.

    What it is

    Anxious dogs pant, pace, drool, tremble, hide, destroy things when left, bark or howl, lose house-training or become aggressive when frightened. Separation anxiety shows within minutes of the owner leaving; noise phobia around storms and fireworks; situational anxiety at the vet, the groomer or in the car. Some dogs are anxious in general. Pain and medical illness can look like anxiety, so the vet checks first.

    Treatment rests on behaviour modification: desensitisation and counter-conditioning taught by the vet or a qualified behaviourist, predictable routines, exercise and enrichment. Medicines reduce the fear enough for the training to work, or get the dog through an event; they are not a substitute for the plan.

    How it is treated

    For predictable events, as-needed medicines given an hour or two beforehand: trazodone, gabapentin, or both together, and for severe noise phobia a fast-acting veterinary gel (dexmedetomidine) applied to the gums. A trial dose at home before the real event is standard advice.

    For ongoing anxiety, a daily medicine that takes weeks to work: fluoxetine (the human generic, or the veterinary chewable Reconcile) or clomipramine (Clomicalm or the human generic). These are continued for months while training proceeds and then tapered. Benzodiazepines such as alprazolam are used sparingly for specific situations. Pheromone diffusers, calming supplements and pressure wraps help some dogs and are harmless to try.

    What an owner pays cash for

    Almost everything here is a cheap human generic: trazodone, gabapentin, fluoxetine, clomipramine and alprazolam are all filled by any pharmacy, and the first three sit on most flat-price lists, so an anxious dog on a daily SSRI plus event medicine costs its owner mostly dispensing fees. The behaviourist’s time is the real expense. The veterinary chewables (Reconcile, Clomicalm) and the dexmedetomidine gel are brand products bought from the vet.

      Trazodone is a human antidepressant that veterinarians prescribe to calm dogs: after surgery when rest is needed, for thunderstorms and fireworks, for vet visits and grooming, and alongside behaviour therapy for anxiety. It is a cheap generic that any pharmacy can fill.

      Gabapentin is one of the most prescribed medicines in small-animal practice, for chronic pain, nerve pain, seizures and, in cats, for anxiety before vet visits. It is a human generic, so a vet’s prescription can be filled at an ordinary pharmacy, with one important caution about the liquid.

    • FluoxetinePharmacy can fill

      Fluoxetine is the most used daily medicine for separation anxiety, compulsive behaviours and aggression in dogs, and for spraying and anxiety in cats. A veterinary version exists, but the human generic is the same molecule and any pharmacy can fill a vet’s prescription for it.

    • ClomipraminePharmacy can fill

      Clomipramine is a tricyclic antidepressant approved for separation anxiety in dogs as Clomicalm, and used off-label for compulsive behaviours and for spraying in cats. The human generic capsules contain the same drug and can be filled at any pharmacy on a vet’s prescription.
