Mood Stabilizers/Anticonvulsants
Lamotrigine
Lamotrigine (Lamictal, Lamictal XR, Lamictal ODT) is an anticonvulsant labeled for several kinds of epilepsy and, except in its extended-release form, for maintenance treatment of bipolar I disorder in adults. Every form is sold as a generic. It carries a boxed warning about serious, sometimes fatal rashes, so doses rise slowly over several weeks.
This page is general information about a medication and how cash prices for it are set. It is not medical advice and does not replace the label, your prescriber or your pharmacist. How we handle medical information
Each form has generics rated to its own Lamictal brand: tablets and extended-release tablets from many makers, orally disintegrating and dispersible tablets from several. Lamictal 50 and 250 mg tablets and the 100 mg dispersible tablet are discontinued.
About Lamotrigine
Lamotrigine is an anticonvulsant; MedlinePlus says it works by decreasing abnormal electrical activity in the brain. Lamictal and its generics are labeled as add-on therapy for partial-onset seizures, primary generalized tonic-clonic seizures and the seizures of Lennox-Gastaut syndrome from age 2; for switching to lamotrigine alone from age 16 when partial-onset seizures are treated with carbamazepine, phenytoin, phenobarbital, primidone or valproate alone; and for maintenance treatment of bipolar I disorder in adults, to delay mood episodes, not to treat an acute manic or mixed episode. Lamictal XR is labeled only for epilepsy, from age 13. Doses rise in steps one to two weeks apart, on a schedule set by the other medicines taken; in bipolar disorder without interacting drugs, 25 mg a day for two weeks, 50 mg for two weeks, then 100 mg, and 200 mg by week six.
The boxed warning says lamotrigine can cause serious rashes needing hospital care, including Stevens-Johnson syndrome and, rarely, toxic epidermal necrolysis or death. Risk is higher in children, with valproate, when the starting dose or the pace of increases is exceeded, and, as an October 2025 update added, with the HLA-B*1502 gene variant, linked to higher risk in studies of patients of certain Asian ancestry. Nearly all life-threatening rashes began within 2 to 8 weeks; because harmless and serious rashes cannot be reliably told apart, it is ordinarily stopped at the first sign of rash unless the rash is clearly unrelated. Other warnings cover HLH, an overactive immune reaction that typically starts with a persistent fever; DRESS and organ failure; heart rhythm problems in people with significant structural or functional heart disease; low blood counts; aseptic meningitis; suicidal thoughts and behavior; and seizures on sudden stopping. Dizziness, headache, double vision, unsteadiness, nausea and sleepiness are common side effects.
Each form is a separate product, and starter and titration kits hold the first five weeks of doses, matched to the other medicines taken. Because the names Lamictal and lamotrigine have been confused with other drugs, the label advises checking the tablets at every fill. Estrogen-containing birth control lowers lamotrigine levels, so doses usually change when it is started or stopped, and levels also fall during pregnancy. Pregnancy registries and studies have not found a higher rate of major birth defects; lamotrigine passes into breast milk, and breastfed infants are watched for rash, pauses in breathing, drowsiness and poor sucking.
Taking Lamotrigine
- How to take it
- Once or twice a day (extended-release once a day), with or without food, exactly as scheduled. Swallow extended-release tablets whole. Tablets for oral suspension may be swallowed, chewed, or dispersed in a little water or diluted fruit juice and all of it drunk at once; move a disintegrating tablet around on the tongue until it breaks up.
- If you miss a dose
- Take it when you remember, unless the next dose is almost due; then skip it, and never take a double dose. After a longer break, ask the prescriber before restarting: past five half-lives (a span that depends on your other medicines), the label calls for the initial schedule again.
- Storage
- At room temperature, away from heat and moisture, out of reach of children. If it comes in a blister pack, do not use it if blisters are torn, broken or missing.
- Interactions to know about
- Valproate raises lamotrigine levels and the rash risk, so doses are lower. Carbamazepine, phenytoin, phenobarbital, primidone, rifampin, lopinavir/ritonavir and estrogen-containing birth control lower its levels, so doses change when any is started or stopped. Other sodium channel blockers may add to the heart rhythm risk.
This summarizes the official prescribing information. Your prescriber’s and pharmacist’s instructions, and the leaflet that comes with your medicine, take priority.
Why the cash price for Lamotrigine differs from pharmacy to pharmacy
Generic lamotrigine tablets and extended-release tablets have many makers, the orally disintegrating and dispersible tablets fewer. No form can be swapped at the counter for another, so each is quoted in the form written; a prescription that names a Lamictal brand and blocks substitution is priced as a brand, and kits are quoted as kits.

Ways to pay less for Lamotrigine without insurance
- Ask for generic lamotrigine in the form your prescription names, and check that it does not name a Lamictal brand only.
- If a starter kit is prescribed, ask for a quote on a generic kit matched to your other medicines.
- Once your dose is stable, ask for a ninety-day supply and whether lamotrigine is on the pharmacy’s flat-price generic list.
- If extended-release or orally disintegrating tablets are prescribed, ask the prescriber whether regular tablets would suit you.
- Upload the prescription to Script Unlock so licensed pharmacies can quote their cash price. You pay the pharmacy directly.
