    About Cariprazine

Cariprazine is an atypical antipsychotic taken once a day, with or without food. Its label covers schizophrenia in adults and children 13 and older; acute manic or mixed episodes of bipolar I disorder in adults and children 10 and older; and, in adults only, depressive episodes of bipolar I disorder and add-on treatment with an antidepressant for major depressive disorder. The pediatric uses and the 0.5 and 0.75 mg capsules were added in December 2025. Adults with schizophrenia or mania start at 1.5 mg, may go to 3 mg on the second day, and take up to 6 mg; for bipolar depression and the depression add-on the start is 1.5 mg, with 3 mg possible from day 15 and no more than 3 mg. Children start at 0.5 mg, build up over about a week, and take no more than 4.5 mg.

The boxed warnings say older people with dementia-related psychosis treated with antipsychotics have a higher risk of death, and cariprazine is not approved for them; and that antidepressants increased suicidal thoughts and behaviors in children, adolescents and young adults, so patients are monitored for worsening and suicidal thinking. Other warnings cover stroke in older people with dementia, neuroleptic malignant syndrome, tardive dyskinesia, side effects that appear late, weight gain and rises in blood sugar and cholesterol, low white blood cell counts, dizziness or fainting on standing, falls, seizures, impaired thinking and motor skills, overheating and trouble swallowing. Weight is checked at the start and often afterward, with fasting blood sugar and cholesterol tests at the start and periodically. Anyone who has had an allergic reaction to it must not take it. Movement symptoms and restlessness (akathisia) are among the most common side effects for every use in adults, joined by indigestion, vomiting, sleepiness, nausea, tiredness, constipation, trouble sleeping, increased appetite or dizziness depending on the use; side effects in children were generally similar.

Cariprazine and its active metabolites build up slowly, so a dose change is not fully reflected in blood levels for several weeks, and side effects can first appear weeks after starting or raising the dose; the label asks prescribers to watch for side effects and response for several weeks after each change. Strong or moderate CYP3A4 inhibitors call for a lower dose, and CYP3A4 inducers are not recommended with it. It is not recommended in severe kidney or liver impairment, and it is not a controlled substance. Animal data suggest it may harm a fetus, babies exposed late in pregnancy may have movement or withdrawal symptoms, and a pregnancy registry follows outcomes; whether it passes into human breast milk has not been studied.

    Taking Cariprazine

    How to take it
Once a day, with or without food, following the build-up schedule the prescriber gives. Because it builds up slowly, effects and side effects can take weeks to settle after a start or a dose change, and side effects can last for several weeks after stopping.
    If you miss a dose
Take it as soon as you can, but skip the missed dose if it is almost time for the next one. Do not take two doses at once.
    Storage
At room temperature, 68°F to 77°F (20°C to 25°C), out of reach of children; protect the 3 mg and 4.5 mg capsules from light to help keep the color from fading.
    Interactions to know about
Strong or moderate CYP3A4 inhibitors raise its levels and call for a lower dose, and CYP3A4 inducers are not recommended with it, so give the pharmacist a full list of your medicines. Avoid alcohol and becoming overheated or dehydrated, and do not drive until you know how it affects you.

This summarizes the official prescribing information. Your prescriber’s and pharmacist’s instructions, and the leaflet that comes with your medicine, take priority.

Why the cash price for Cariprazine differs from pharmacy to pharmacy

Cariprazine has no generic, so every pharmacy dispenses the same brand capsule and quotes differ only by each pharmacy’s purchase cost, markup and dispensing fee. A pharmacy that rarely fills it may need to order it, particularly the newer 0.5 and 0.75 mg capsules. Each usual daily dose matches a single capsule strength, so a prescription normally needs only one capsule a day.

Ways to pay less for Cariprazine without insurance

    • Ask the maker whether its savings or patient assistance program applies to someone paying cash; many copay cards require commercial insurance.
    • Check that the prescription is written as one capsule of your daily dose rather than two smaller capsules.
    • Once your dose is stable, ask for a ninety-day supply, and compare more than one pharmacy.
    • If cost is a barrier, ask the prescriber whether a generic medicine such as aripiprazole would suit you.
    • Upload the prescription to Script Unlock so licensed pharmacies can quote their cash price. You pay the pharmacy directly.
