About Rimegepant

Rimegepant blocks the receptor for calcitonin gene-related peptide (CGRP), a messenger involved in migraine pain. The label covers two uses in adults: the acute treatment of migraine with or without aura, and, since May 2021, the preventive treatment of episodic migraine. For an attack the dose is one 75 mg tablet as needed, with no more than 75 mg in 24 hours, and the label notes that the safety of more than 18 doses in 30 days has not been established. For prevention the dose is 75 mg every other day. It can be taken with or without food, and its safety and effectiveness in children have not been established.

There is no boxed warning. It must not be taken by anyone who has had an allergic reaction to rimegepant or an ingredient of the tablet; reactions have included anaphylaxis, shortness of breath and rash, and serious ones can appear days after a dose. New or worsening high blood pressure has been reported since it came to market, most often within 7 days of starting and sometimes needing treatment or a hospital stay, so blood pressure is monitored. New or worsening Raynaud’s phenomenon (reduced blood flow to the fingers or toes) has also been reported, and the drug is stopped if it appears. Nausea was the most common side effect in the study of acute treatment; in prevention, nausea and stomach pain or indigestion were the most common.

The tablet is lifted from its peeled-back blister with dry hands, never pushed through the foil, and dissolves on or under the tongue without water; it is not kept outside the blister for later. Strong CYP3A4 inhibitors such as itraconazole, and strong or moderate CYP3A inducers such as rifampin, are avoided. With a moderate CYP3A4 inhibitor such as fluconazole, or a potent P-gp inhibitor such as amiodarone, cyclosporine, lapatinib, quinidine or ranolazine, another dose is not taken within 48 hours. It is avoided in severe liver impairment and in end-stage kidney disease, and a pregnancy registry collects outcomes. The label’s dosing sections were revised in March 2026, and its contraindication and allergy warning in August 2025.

Taking Rimegepant

How to take it

With dry hands, peel back the foil of one blister and lift the tablet out; do not push it through the foil. Place it on or under the tongue to dissolve; no water is needed. Take it right after opening the blister, with or without food, and never more than one tablet in 24 hours.

If you miss a dose

For attacks it is taken only when needed, so there is no missed dose to make up. Neither the label nor MedlinePlus gives missed-dose instructions for every-other-day prevention, so ask the pharmacist how to get back on schedule, and never take more than one tablet in 24 hours.

Storage

At room temperature, 68°F to 77°F (20°C to 25°C), out of reach of children. Keep each tablet in its blister until you take it; do not store it outside the blister for later use.

Interactions to know about

Itraconazole and other strong CYP3A4 inhibitors, and rifampin and other strong or moderate CYP3A inducers, are avoided. With fluconazole or another moderate CYP3A4 inhibitor, or a potent P-gp inhibitor such as amiodarone, cyclosporine, lapatinib, quinidine or ranolazine, do not take another dose within 48 hours.
