ACE Inhibitors
Ramipril
Ramipril is an ACE inhibitor for high blood pressure, for lowering the risk of heart attack, stroke and cardiovascular death in people 55 and older at high risk, and for heart failure after a heart attack. The Altace brand and the Vostally oral solution have both been discontinued, so every fill is a generic, and it carries a boxed warning against use in pregnancy.

Generic capsules are sold in all four strengths, and a generic tablet is listed too; the Orange Book lists Altace capsules and tablets and the Vostally oral solution as discontinued.
About Ramipril
The liver converts ramipril to ramiprilat, which blocks the enzyme that makes angiotensin II, a hormone that narrows blood vessels and makes the body hold on to salt and water. The label covers high blood pressure, alone or with a thiazide diuretic; lowering the risk of heart attack, stroke or cardiovascular death in people 55 or older at high risk because of a history of coronary artery disease, stroke or peripheral vascular disease, or diabetes with at least one other risk factor; and heart failure in stable patients in the first days after a heart attack. Its safety and effectiveness in children are not established. For blood pressure it usually starts at 2.5 mg once a day (usual range 2.5 to 20 mg a day); for risk reduction it builds from 2.5 mg to 10 mg once a day over about a month; after a heart attack it starts at 2.5 mg twice a day, aiming for 5 mg twice a day. Poor kidney function calls for lower doses.
The boxed warning is fetal toxicity: drugs that act on the renin-angiotensin system can injure and kill a developing baby, so ramipril is stopped as soon as pregnancy is detected. In February 2026 the angioedema warning was revised to say this swelling can occur at any time during treatment. It can affect the face, lips, tongue or throat, where it can be fatal and needs emergency care, or the intestine, causing stomach pain; controlled trials found it more often in Black patients, and the risk may be higher after angioedema from another cause or with an mTOR inhibitor or a neprilysin inhibitor. A dry, persistent cough can occur and clears after stopping. Ramipril can drop blood pressure too far, especially at the start in people low on salt or fluid, raise potassium, worsen kidney function and, rarely, cause liver injury or a fall in white blood cells. Headache, dizziness, tiredness and cough are the most common side effects.
It must not be taken by anyone allergic to ramipril or another ACE inhibitor (including angioedema with one) or with a history of hereditary or idiopathic angioedema; nor within 36 hours of switching to or from sacubitril-valsartan (Entresto), or with aliskiren in diabetes. Potassium and kidney function are checked, and the label advises against use while breastfeeding. Vostally, a 1 mg per mL oral solution approved in July 2025, is listed as discontinued along with Altace, so pharmacies dispense generic capsules or tablets.
Taking Ramipril
- How to take it
- Once or twice a day as prescribed, with or without food, at about the same times each day. Swallow the capsule whole, or open it and mix the contents with about 4 ounces of applesauce, water or apple juice and take all of it; a prepared mixture keeps up to 24 hours at room temperature or 48 hours in the refrigerator. Keep taking it even if you feel well.
- If you miss a dose
- Take it as soon as you remember, unless it is almost time for the next dose; then skip the missed dose and carry on as usual. Do not take a double dose.
- Storage
- At room temperature in the original container, tightly closed, away from heat and moisture, out of reach of children. Use a take-back program for unused medicine rather than flushing it.
- Interactions to know about
- Potassium supplements, potassium salt substitutes and potassium-sparing diuretics can raise potassium too high; ibuprofen, naproxen and other NSAIDs can weaken its effect and strain the kidneys; it raises lithium levels; and starting it on top of a diuretic can drop blood pressure too far, as can diarrhea, vomiting or heavy sweating. mTOR inhibitors such as temsirolimus add to the risk of angioedema.
This summarizes the official prescribing information. Your prescriber’s and pharmacist’s instructions, and the leaflet that comes with your medicine, take priority.
Why the cash price for Ramipril differs from pharmacy to pharmacy
Generic ramipril capsules cost pharmacies little, so a quote is mostly the dispensing fee and markup, and whether ramipril is on a pharmacy’s flat-price generic list varies. With no brand left on the market, quotes for the same capsule differ only in how each pharmacy prices it; capsules and tablets are different dosage forms, so a pharmacy fills the form written.
Twice-daily dosing after a heart attack, or a dose written as two capsules, raises the count where a pharmacy prices per capsule, and each new strength while the dose is raised is a separate fill and fee.

Ways to pay less for Ramipril without insurance
- Ask whether ramipril is on the pharmacy’s flat-price generic list, and compare more than one pharmacy.
- Once your dose is settled, ask for a ninety-day supply.
- If your dose is 10 mg, check that it is written as one 10 mg capsule rather than two 5 mg capsules.
- If it is quoted high, ask the prescriber whether another generic ACE inhibitor, such as lisinopril, would suit you.
- Upload the prescription to Script Unlock so licensed pharmacies can quote their cash price. You pay the pharmacy directly.
