About Pantoprazole
Pantoprazole reduces stomach acid by the same mechanism as omeprazole. It is approved for short-term treatment of erosive esophagitis caused by reflux, for maintaining healing, and for conditions with acid overproduction. It has less interaction with the liver enzyme that activates clopidogrel than omeprazole does, which is why prescribers favor it for people on that antiplatelet drug.
It is taken once daily, ideally before a meal, and can take a few days to reach full effect. The same long-term cautions as other proton pump inhibitors apply: magnesium and vitamin B12 levels, bone health, and a modest increase in gut infections. Prescribers periodically reassess whether ongoing treatment is needed.
Pantoprazole is not sold over the counter in the United States, so unlike omeprazole there is no non-prescription route to the same molecule.
Taking Pantoprazole
- How to take it
- Once a day, usually 30 to 60 minutes before breakfast. Swallow the delayed-release tablet whole; do not crush, split or chew it. The delayed-release granules are sprinkled on a spoonful of applesauce or mixed in apple juice and taken straight away.
- If you miss a dose
- Take it as soon as you remember, unless it is nearly time for your next dose; in that case skip the missed dose and carry on as usual. Never take two doses at once to make up.
- Storage
- At room temperature in the original container, away from heat and moisture (so not in a bathroom cabinet), and out of reach of children.
- Interactions to know about
- It reduces the absorption of medicines that need stomach acid, including some HIV medicines (it must not be taken with rilpivirine), some antifungals and iron, and high-dose methotrexate levels can rise. Long-term use can lower magnesium and vitamin B12. It has little effect on clopidogrel, which is why it is often chosen for people taking that blood thinner.
