Ezetimibe (generic Zetia) is a once-daily tablet that lowers LDL cholesterol by blocking its absorption in the gut. It is usually added to a statin, or used alone when a statin is not tolerated, and has been a cheap generic since 2016.

About Ezetimibe
Ezetimibe works differently from statins: instead of reducing cholesterol production in the liver, it blocks the transporter that absorbs cholesterol from food and bile in the small intestine. On its own it lowers LDL modestly; added to a statin it adds a further meaningful reduction, and the combination has outcome evidence in people who have had a heart attack.
It is taken once a day at any time, with or without food, and has very few side effects; muscle symptoms are rare and it has fewer drug interactions than most statins. It is often the next step when a statin alone does not reach the LDL target, or the first alternative when a patient cannot take a statin.
Fixed-dose combinations with simvastatin and with rosuvastatin exist, and a combination with bempedoic acid is a newer brand product.
Why the cash price for Ezetimibe differs from pharmacy to pharmacy
Ezetimibe is a commodity generic; the cash price is fee and margin. Because it is almost always taken with a statin, two prescriptions and two dispensing fees are the norm, so the pharmacy’s fee structure matters.
