Overview
Triptans narrow the dilated blood vessels around the brain and block pain signalling in the trigeminal nerve. Taken early in an attack they stop or shorten it for most people; they are not preventives and do not help tension headache. Sumatriptan is the original and comes as tablets, nasal spray and injection; rizatriptan and zolmitriptan come as dissolving tablets useful with nausea; eletriptan and others differ in speed and duration.
Tingling, flushing, tightness in the chest or throat and drowsiness are the common effects. Because they narrow blood vessels they are not used in coronary artery disease, uncontrolled high blood pressure or after a stroke. Using them on many days a month causes medication-overuse headache, so prescribers limit quantities and consider a preventive for frequent attacks.
Newer classes, the gepants and ditans, work differently and are brand-priced; they are options for people who cannot take triptans.
How the drugs in this class differ when you pay cash
Every common triptan is generic, but prescriptions are small, typically nine tablets a month, so the dispensing fee dominates and chains often price the same nine tablets far above independents. Nasal sprays and especially the sumatriptan injection cost pharmacies much more per dose. Dissolving tablets cost more than plain tablets.
A prescription for a larger quantity per fill, where the prescriber agrees, spreads the fee; a plain tablet instead of a dissolving one, where nausea allows, cuts the drug cost.
When paying cash beats a copay, and when it does not
For triptan tablets a cash quote from a flat-price or independent pharmacy is usually at or below a copay. For sprays, injections and the newer brand classes, insurance or manufacturer programmes usually win.
