Overview
Serotonin-norepinephrine reuptake inhibitors are antidepressants with a second action on norepinephrine, which gives them a role in pain conditions and in anxiety as well as depression. Venlafaxine is used for depression, generalized and social anxiety and panic disorder; duloxetine for depression, anxiety, diabetic nerve pain, fibromyalgia and chronic musculoskeletal pain; desvenlafaxine is venlafaxine’s active metabolite.
They take weeks to work, can raise blood pressure at higher doses, and have noticeable discontinuation symptoms if doses are missed or stopped abruptly, venlafaxine especially. Nausea, sweating, sleep changes and sexual side effects are the usual complaints.
Like all antidepressants they carry the boxed warning about suicidal thinking in young people.
How the drugs in this class differ when you pay cash
Venlafaxine extended-release capsules and duloxetine capsules are cheap generics. The venlafaxine extended-release tablet is a separate, later product with few manufacturers and a much higher price, and desvenlafaxine, though generic, costs pharmacies more than either. A prescription written for the venlafaxine tablet or for desvenlafaxine can cost many times what the venlafaxine capsule does for a similar effect.
Brand Effexor XR, Cymbalta and Pristiq add nothing for a cash-paying patient. Duloxetine doses above the usual starting dose are made up of two capsules, which doubles the count but not the fee.
Duloxetine capsules come in several strengths and some pharmacies stock only the most common; a prescription written for an unusual strength may be quoted higher because it has to be ordered. Levomilnacipran and milnacipran are newer SNRIs that remain brand-priced and are prescribed less often.
When paying cash beats a copay, and when it does not
For venlafaxine capsules and duloxetine, cash from a flat-price, independent or mail-order pharmacy is usually at or below a copay. For venlafaxine tablets and desvenlafaxine, compare carefully, and ask the prescriber whether the capsule form would do.
