Erectile Dysfunction: medications and paying cash
ED medications improve blood flow to help achieve and maintain erections when sexually stimulated.
Overview
Erectile dysfunction is common, increasingly so with age, and is often an early sign of vascular disease, diabetes or low testosterone, so a prescriber will usually check blood pressure, blood sugar and sometimes hormone levels before or alongside treatment. Medication, counseling and treating the underlying cause work together.
The PDE5 inhibitors increase blood flow in response to sexual stimulation. Sildenafil works for a few hours and is taken as needed; tadalafil lasts much longer and can be taken daily at a low dose, which also treats urinary symptoms from an enlarged prostate. Vardenafil and avanafil are alternatives with similar effects. None of them works without stimulation, and all are dangerous with nitrate heart medicines.
Side effects are headache, flushing, nasal congestion and indigestion; a sudden change in vision or hearing, or an erection lasting more than a few hours, needs urgent care.
Choosing between the options when you pay cash
Sildenafil and tadalafil are commodity generics and the cash price of either is mostly the pharmacy’s fee and mark-up, which varies enormously for these drugs in particular. Two details matter. The 20 mg sildenafil tablet, approved for pulmonary hypertension, is the same molecule as the erectile dysfunction strengths and some prescribers use it; and quantity limits, which insurers impose, do not apply to a cash purchase, so a prescription for more tablets per fill spreads the dispensing fee. Daily low-dose tadalafil is a different dosing pattern with its own quote.
Brand Viagra and Cialis are the same drugs at a far higher price. Telehealth services often bundle a consultation with a markup on the medicine; a prescription from your own doctor filled at a pharmacy that quotes competitively is usually cheaper.
When paying cash beats a copay, and when it does not
