    Trazodone vs zolpidem for sleep: compared

    Zolpidem (Ambien) is approved for insomnia and works quickly, but it is a controlled substance with a warning about sleepwalking and sleep-driving. Trazodone is an antidepressant often prescribed off-label at low doses for sleep. For long-term insomnia, guidelines recommend cognitive behavioral therapy before either.

    For insomnia lasting months, US guidelines recommend cognitive behavioral therapy for insomnia, known as CBT-I, as the first treatment: it works as well as medicine in the short term and better in the long run, with no side effects. It is available through clinicians, apps and online programs. Medicines are best for short periods or when CBT-I is not enough.

    Zolpidem

    Zolpidem helps people fall asleep within about half an hour. It carries a boxed warning about complex sleep behaviors, such as sleepwalking or driving while not fully awake, which can cause serious injury; anyone who experiences them should stop it. Women are prescribed lower doses because levels stay higher the next morning. It can cause dependence, so it is not stopped suddenly after regular use, and it must never be combined with alcohol.

    Trazodone

    Trazodone is not a controlled substance and is not habit-forming in the way sleeping pills can be, which is why it is often chosen, especially when depression is also present. The evidence for it in insomnia without depression is limited. It can cause grogginess, dizziness when standing and, rarely, a prolonged painful erection that needs emergency care, and it adds to the effects of other serotonin-affecting medicines.

    Older adults

    Both raise the risk of falls, confusion and next-day drowsiness in older adults, especially when getting up at night. Guidelines advise particular caution with sleeping pills at older ages, lower doses are used, and non-drug approaches are tried first.
