    Hydroxyzine vs buspirone for anxiety: compared

    Hydroxyzine and buspirone are both non-addictive alternatives to benzodiazepines for anxiety, but they work in opposite ways. Hydroxyzine works within an hour and is used as needed; buspirone must be taken every day and takes two to four weeks to help.

    How each fits

    Hydroxyzine suits short spells of anxiety or trouble sleeping because of anxiety, since it works quickly; its drowsiness can help at night but gets in the way during the day. Buspirone is a long-term treatment for generalized anxiety disorder that builds up over weeks and is not useful as needed. Some people take buspirone daily with hydroxyzine for occasional difficult moments, under their prescriber’s direction.

    Cautions

    Hydroxyzine can affect heart rhythm, so care is needed with other medicines that do, and older adults are more prone to confusion, constipation and falls with it. Buspirone can cause dizziness, nausea and headache at first; it is not combined with MAOIs, and grapefruit juice and some antifungals raise its levels. Both add to the effects of alcohol and other sedatives.

    Compared with other options

    For lasting anxiety, SSRIs such as sertraline and talking therapies are the main treatments, and benzodiazepines are generally reserved for short periods because of dependence. Hydroxyzine and buspirone are useful when those are not suitable or need support.

    Cost

    Both are inexpensive generics. Hydroxyzine comes as tablets, capsules and a liquid; ask for a quote on the form prescribed.

    Missed doses and food

    Hydroxyzine taken as needed has no missed doses; on a schedule, take it when you remember unless the next dose is near. Take a missed buspirone dose when you remember unless the next one is near, and keep taking buspirone the same way each day with or without food, because food changes how much of it is absorbed.
