Strattera vs Ritalin for ADHD: non-stimulant vs stimulant

Strattera (atomoxetine) is a non-stimulant ADHD medicine that works around the clock but takes weeks to reach full effect; Ritalin (methylphenidate) is a stimulant that works within an hour. Atomoxetine is not a controlled substance, which matters for some families and some adults.

    When a non-stimulant makes sense

Stimulants are usually the first choice because they work quickly and well for most people. Atomoxetine is often chosen when stimulants cause intolerable side effects, when there is a concern about stimulant misuse in the household, or for some people with anxiety or tics. Its steady effect also covers evenings and early mornings.

    Side effects

Atomoxetine can cause stomach upset, reduced appetite, tiredness and, in adults, urinary or sexual side effects; it carries a warning about suicidal thoughts in young people, and rarely liver injury, so report yellowing skin or dark urine. Methylphenidate commonly reduces appetite and can disturb sleep. Both can raise heart rate and blood pressure.

    Taking them

Atomoxetine capsules are swallowed whole and never opened, because the contents irritate the eyes; it is taken once or twice a day, every day. Methylphenidate is taken in the morning, with some forms repeated midday, and late doses are avoided to protect sleep.

    Interactions

Neither is taken with an MAOI. Fluoxetine and paroxetine raise atomoxetine levels, so its dose is adjusted. Blood pressure medicines and other stimulants need care with methylphenidate.

    Cost

Both are generics. Prices vary with the form and between pharmacies, so compare quotes for the exact product prescribed.

    Missed doses

For atomoxetine, take a missed dose when you remember, but never more than the total daily dose in 24 hours. For methylphenidate, skip a missed morning dose if it is already late in the day, to protect sleep.

    Questions for your prescriber

    • Is there a reason to avoid a stimulant for me or my child?
    • How long should we wait to judge atomoxetine?
    • How will heart rate, blood pressure and mood be monitored?
