Apoquel vs prednisone for itchy dogs: compared

Apoquel (oclacitinib) and prednisone both relieve allergic itch in dogs quickly. Apoquel targets the itch signal and avoids most steroid side effects, which makes it suited to long-term use; prednisone is inexpensive and powerful but causes thirst, hunger and longer-term problems.

    How they work

Prednisone calms the whole immune system, which stops itch and inflammation broadly but affects many body systems. Apoquel blocks the specific signals that drive allergic itch, so it relieves itch with fewer of the side effects of steroids.

    Side effects over time

Short steroid courses mainly cause thirst, frequent urination, hunger and panting. Long-term prednisone can lead to weight gain, muscle loss, skin thinning, infections, diabetes and liver changes, so vets taper to the lowest effective dose. Apoquel can make dogs more prone to infections and skin mites and is not used in dogs under 12 months or with serious infections; your vet will monitor dogs on it long term.

    Finding the cause

Neither medicine cures allergies. Flea control, food trials, treating skin infections and managing environmental allergies, including allergy shots, are part of a lasting plan. Cytopoint, a monthly injection, is another option.

    Cats

Cats are usually given prednisolone rather than prednisone because they convert prednisone poorly. Apoquel is not approved for cats.

    Cost

Prednisone is an inexpensive human generic a regular pharmacy can fill. Apoquel costs more and comes through your vet or a veterinary pharmacy.

    Missed doses

Give a missed dose when you remember unless the next one is near, and never double up. Do not stop prednisone suddenly after more than a short course; your vet will taper it.

    Questions for your vet

    • What is causing my dog’s itch?
    • Is a short steroid course or long-term Apoquel the better plan?
    • What monitoring does long-term treatment need?
