Trulicity vs Ozempic: weekly diabetes injections compared
Trulicity (dulaglutide) and Ozempic (semaglutide) are once-weekly GLP-1 injections for type 2 diabetes that also lower heart risks. In a head-to-head trial Ozempic lowered blood sugar and weight more, while Trulicity’s pen hides the needle and needs no setup.

How they compare in practice
Both slow stomach emptying, boost insulin release when blood sugar is high and reduce appetite, and both cause similar stomach side effects, mainly when starting and after dose increases. In the SUSTAIN 7 trial, semaglutide lowered A1c and weight more than dulaglutide at the doses studied. Trulicity is the only one of the two approved for children with type 2 diabetes, and its single-use pen is simpler for people who dislike handling needles.
Weight and coverage
Neither is approved for weight loss on its own; semaglutide for weight management is sold as Wegovy. Insurance usually covers both for type 2 diabetes, often with prior authorization, and Ozempic is among the drugs with a Medicare negotiated price from January 2027.
Switching
Doses are not equivalent between the two, so a switch usually restarts at a lower dose of the new drug, chosen by your prescriber, to limit stomach side effects.
Missed doses and storage
A missed Trulicity dose can be taken if at least 3 days remain before the next one, and a missed Ozempic dose within 5 days; otherwise skip it. Trulicity pens can be kept at room temperature for up to 14 days, and an Ozempic pen in use for up to 56 days.
