    Cymbalta vs Effexor: SNRI antidepressants compared

    Cymbalta (duloxetine) and Effexor XR (venlafaxine) are both SNRIs, acting on serotonin and norepinephrine. Duloxetine is also approved for several pain conditions; venlafaxine covers more anxiety disorders but raises blood pressure more and causes stronger withdrawal symptoms if stopped suddenly.

    Pain or anxiety

    Duloxetine’s approvals for nerve pain from diabetes, fibromyalgia and chronic back or arthritis pain make it a common choice when depression or anxiety comes with chronic pain. Venlafaxine is approved for a wider range of anxiety disorders, including social anxiety and panic disorder, and is sometimes used for hot flashes off-label.

    Safety differences

    Venlafaxine can raise blood pressure, so it is checked regularly. Duloxetine is avoided in people with significant liver disease or heavy alcohol use, because of a risk of liver injury, and fluvoxamine and ciprofloxacin raise its levels sharply. Both carry the risks of serotonin medicines, including serotonin syndrome with other serotonin-affecting drugs and more bleeding with blood thinners, and both carry a warning about suicidal thoughts in young people.

    Stopping

    Both can cause discontinuation symptoms, such as dizziness, irritability, nausea and electric-shock sensations, if stopped suddenly. Venlafaxine leaves the body quickly, so its symptoms can be stronger and appear within a day of a missed dose; both are reduced gradually.

    Cost

    Both are generics. Prices vary with the form and strength, so ask for a quote on the exact capsule prescribed.

    Missed doses

    Take a missed dose when you remember unless the next one is near, and never double up. Because venlafaxine leaves the body quickly, even one missed dose can bring dizziness or irritability, so keeping to a daily routine matters more than with many antidepressants.
