    Cipro vs Macrobid for a urinary tract infection: compared

    Macrobid (nitrofurantoin) is a first-line treatment for uncomplicated bladder infections. Cipro (ciprofloxacin) is stronger and also treats kidney infections, but because of serious side effects the FDA advises reserving it for bladder infections when other options are not suitable.

    Why Macrobid usually comes first

    Nitrofurantoin concentrates in the urine, works against most bacteria that cause bladder infections, and has kept its effectiveness because it is used only for this. Ciprofloxacin is a powerful broad antibiotic, and in 2016 the FDA advised that fluoroquinolones such as Cipro be reserved for uncomplicated bladder infections only when no other option is suitable, because their side effects can be disabling.

    When Cipro is the right choice

    Nitrofurantoin does not reach the kidneys, so it does not treat a kidney infection, signaled by fever, flank pain, nausea or feeling very unwell. Ciprofloxacin and some other antibiotics do. Nitrofurantoin is also avoided when kidney function is significantly reduced.

    Side effects to know

    Stop Cipro and seek advice at the first sign of tendon pain or swelling, numbness or tingling, or mood changes, and take it apart from antacids, calcium and iron. Nitrofurantoin can turn urine brown, which is harmless; stop it and seek advice for cough, chest pain or breathlessness. Both are taken for the full course.

    Cost

    Both are inexpensive generics. A urine culture can show which antibiotic will work if the infection does not clear.

    Preventing repeat infections

    Drinking enough fluids and urinating after sex are commonly advised to lower the chance of another bladder infection. If infections keep returning, ask about testing and prevention options rather than repeating the same antibiotic each time.
