    About Lisinopril

    Lisinopril blocks the enzyme that converts angiotensin I to angiotensin II, a hormone that narrows blood vessels and prompts the body to retain salt and water. Blocking it relaxes blood vessels and lowers blood pressure, reduces strain on a weakened heart, and slows kidney damage in people with diabetes, which is why it is prescribed for several different reasons.

    It is taken once daily and works over a full day. A dry, persistent cough affects a meaningful minority of people and is the usual reason for switching to an ARB such as losartan, which works on the same system without the cough. Dizziness when standing can occur, especially after the first doses or with dehydration. Potassium levels and kidney function are checked after starting.

    Lisinopril is not used during pregnancy. It is also sold in a fixed combination with hydrochlorothiazide, and as an oral solution for people who cannot swallow tablets.

    Why the cash price for Lisinopril differs from pharmacy to pharmacy

    Lisinopril costs pharmacies almost nothing to stock, so the cash price is dispensing fee plus margin. The percentage spread between a chain and an independent for the same thirty tablets can be very large even though the dollar amounts are small, and over a lifetime of monthly fills it adds up.

    The combination tablet with hydrochlorothiazide is also generic and usually cheap, but two separate prescriptions for the components sometimes cost less at a given pharmacy than the combination product, or the reverse. It depends on which the pharmacy buys in volume.

    • Ask for a ninety-day supply; for a stable blood pressure dose this removes two dispensing fees a quarter.
    • If you take lisinopril and hydrochlorothiazide, ask the pharmacy to quote the combination tablet and the two separate tablets so you can see which is cheaper there.
    • Ask whether the pharmacy has a flat-price generic list; lisinopril is on many of them.
