    Tirzepatide

    Tirzepatide is a once-weekly injectable that activates both GIP and GLP-1 receptors. It is sold as Mounjaro for type 2 diabetes and as Zepbound for chronic weight management and obstructive sleep apnoea in adults with obesity.

    No generic tirzepatide exists. Compounded tirzepatide is not a generic and is not FDA-approved; the FDA declared the tirzepatide shortage resolved, which limits when compounders may produce it.

    About Tirzepatide

    Tirzepatide works on two gut-hormone receptors at once. Like the GLP-1 drugs it increases insulin release when blood sugar is high, slows stomach emptying and reduces appetite; the added GIP activity is thought to contribute to the larger average weight loss seen in trials compared with single-receptor GLP-1 drugs. It is injected under the skin once a week.

    Mounjaro and Zepbound are the same molecule at the same strengths, approved for different uses. Mounjaro is approved for type 2 diabetes. Zepbound is approved for weight management in adults with obesity, or overweight with a weight-related condition, and for moderate to severe obstructive sleep apnoea in adults with obesity. A pharmacy fills the product written on the prescription.

    Dosing starts low and increases in steps every few weeks. Gastrointestinal effects are the main reason for stopping. The label carries the same boxed thyroid C-cell warning as the GLP-1 class, and it should not be used by people with a personal or family history of medullary thyroid carcinoma.

    Why the cash price for Tirzepatide differs from pharmacy to pharmacy

    As a brand-only product, the wholesale cost is essentially the same for every pharmacy, so the cash price you are quoted is mostly a decision about margin. Independent pharmacies that want your ongoing weekly refills will often price closer to their cost than a chain will.

    The manufacturer sells Zepbound vials directly to self-pay patients at a published price that is lower than the pen list price. That vial route requires drawing up the dose with a syringe rather than using a pen, and it is only for Zepbound, not Mounjaro. Whether a pharmacy can beat that figure depends on the pharmacy, so it is worth putting your prescription in front of several of them.

    Ways to pay less for Tirzepatide without insurance

    • Confirm with your prescriber whether the prescription is for Mounjaro or Zepbound. The vial self-pay programme is only available for Zepbound.
    • Compare the manufacturer’s self-pay vial price against pharmacy bids for pens, and factor in whether you are comfortable using a vial and syringe.
    • The commercial-insurance savings card can reduce cost dramatically if you have coverage; it cannot be used by cash-only, Medicare or Medicaid patients.
    • Ask about a prescription for the maintenance dose you expect to stay on, so you are not repeatedly paying for a strength you will leave behind.
    • If you are offered compounded tirzepatide, ask whether the pharmacy is a licensed 503A pharmacy or a 503B outsourcing facility, where the active ingredient comes from, and whether any other ingredients are added.
